This is only a brief description of the Travel Assistance and Emergency
Evacuation Benefits and Services contained herein. The Master Certificate
shall provide the only basis for providing Benefits and Services in this
Certificate. The benefits and services are to be provided to Participants
while traveling more than one hundred miles (100) from his or her Permanent
Residence. Trip duration's are limited to 90 days or less. All Benefits
and Services in connection with this Certificate must be pre-approved
and arranged by the Assistance Provider.
HOW TO USE THIS BENEFIT:
Call toll free
(if inside the U.S.) or (Collect if outside of the U.S.) seven days a
week, 24 hours a day, 365 days a year. The service representative at the
assistance center will ask you to provide your Member Number and name
to verify your membership. You must use the Travel Assist number to access
or use any of the benefits and services. For General Membership Matters:
If you have questions regarding your membership, would like to report
an address change or a lost or stolen card, or request a certificate listing
all benefits, provisions and exclusions, call our members only number
provided with your plan documents.
