EXTENDED CARE BENEFIT OVERVIEW:

Extended care is one of the fastest growing segments of the health care delivery system. One out of four Americans will face the dilemma of paying for an extended care service for themselves or a family member. Arranging for long-term care for a loved one can be a very emotional experience. Such care can be very expensive and quickly deplete financial resources.

While the decision regarding long-term care can be a difficult one to make, there is now help available to you. You are eligible to participate in the PriMed Extended Care Network, which offers discounted services through a wide variety of extended care providers. The network includes the following types of providers and services:

Participating in the PriMed Extended Care Network assures you of access to high quality medical providers at a reduced price. Through an arrangement with HealthCare Synergies, Inc. (HSI), you are eligible to receive substantial discounts from more than 3,000 providers of long-term services. The PriMed Extended Care Network is not an insurance product. You are eligible to access this benefit regardless of your age or medical condition. The only restriction is that you cannot receive a discount from a Network from whom you are already receiving services.

NETWORK PARTICIPATION MEANS SUBSTANTIAL SAVINGS:

You can save from 5% to 30% off the usual and customary fees for services from PriMed Network Providers. There is no length of stay requirements to receive these contracted rates.

SELECTING A PROVIDER:

The first step in choosing a provider is to determine exactly what type of service is needed and which provider meets this need. A discussion with the patient's physician will assist you in making the right choice regarding the selection of the proper service and provider. It is recommended that you begin your research early and allow adequate time to contact several providers before making your final decision. A listing of network providers in your area from whom you can obtain a discount is included in your member kit. RECEIVING YOUR NETWORK DISCOUNT:

To obtain the discount, select a participating Network provider and show your ID card to the provider before services are delivered. The billing statement you receive from the provider will reflect the provider's usual and customary fees. Immediately on receipt of each bill, the patient or family member must mail or fax this bill (along with the processing form) to HSI for calculation discount and the amount owed to the provider. An explanation regarding the calculated discount is also sent to the provider. You simply make your payment, within thirty days of the original billing date, to obtain the discount.
