When you carry the Medifile Card, your complete medical history is a quick, confidential phone call away!

No one really likes letting an unfamiliar doctor take control of his or her health care in an emergency—but it happens all the time. We become ill while traveling, an injury leaves us unable to speak for ourselves, or our own doctor just isn’t available.

With the Medifile Card, there’s no need to worry! Just a quick telephone call is all it takes to access your complete medical history, including your emergency contact information—any time, day or night.

The Medifile Card puts privacy first. You’ll feel safer knowing that your comprehensive medical history—and your family’s—is easily available to medical professionals in one secure place. Your information is released only with your permission. In an emergency, if a medical professional calls Medifile for your medical history, an efficient security check protects your privacy.

The Medifile Card Gives You:

• Clear instruction regarding your advance directives, such as a living will.
• Total control, even in emergencies, to assure your wishes are followed.

Identify yourself as a member and request your medical history questionnaire. Once you receive it, complete the questionnaire and return it to Medifile. Within two weeks you will receive a Medifile membership kit which includes a copy of your health profile, a personalized Medifile card, an update form and additional information about the program.
